In order to drive or chaperone on a school sponsored field trip you need to have the following forms on file in the school office. Volunteer Form (copy of official dmv identification required) Form completed after watching district video. Personal Use Automobile Form ( copy of drivers license and auto insurance card required) Adult Chaperone Volunteer Participation (needed each field trip)

Registration InstructionsOpen enrollment for the next school year opens up March 1st. All enrollment forms and information needed to enroll your child can be found on www.srvusd.net . Information can be entered directly on the forms. Once you have entered the information, print the forms and turn them into theschool office between the hours of 8:00am-3:30pm along with the following: · Proof of residency (Picture ID, mortgage statement or rental agreement and utility bill) · Child’s birth certificate or passport · Immunization records and proof of physical Child must be five years of age on or before December 2, 2010. Quick LinksHow do I get money on my child's lunch card?Deposit a check made payable to SRVUSD into the wooden locked box on the office counter. Put childs name, teacher, classroom # on the envelope. The lunch personnel will credit the accounts each morning. OR Call the school office for Students ID#, log in to Mealtime online. Set up account, make a deposit (there is a fee),. It takes several days to credit account.
